What is an E-Statement?
An E-Statement is a way to receive your account statement(s) via email. As soon as your statement(s) is ready, we will let you know by email; it is fast, secure and convenient!

• View your statement immediately after processing!
• Reduce risk of exposing financial information when paper statements are mailed or discarded.
• Choose when and if you want to print your E-Statement.

To start receiving your E-Statement, just complete and submit this enrollment form. You will receive an enrollment confirmation via email. At the end of your statement cycle, watch for an email from FCB that will advise when your statement is ready for viewing through FCB online banking.

Note: Adobe Acrobat Reader, Version 7.0 or greater, is required for on-line viewing.

I understand that I will no longer receive paper statements via postal mail unless I request in writing to discontinue E-Statement delivery. I will also notify First Choice Bank of any change with my email address.
